List at least two potential roles for pharmacists related to ensuring the safe and effective use of new and emerging antithrombotic agents for preventing stroke in patients with AF at both the patient-care and institutional levels. Identify relevant patient characteristics to guide the selection and monitoring of antithrombotic therapy in patients with atrial fibrillation.
